information that he or she needs for your effective treatment. Gateway
Orthodontics provides your healthcare professional with computer systems that
organize and process information relating to you and your treatment. Gateway
Orthodontics also uses that information to provide business and consulting
services to your healthcare professional that in turn helps the affiliated healthcare
professional make your treatment experience efficient.
Your
personal information will only be provided to your healthcare professional for
use in connection with your treatment and the operation of the healthcare
professionalÕs practice and will be used by Gateway Orthodontics to provide its
consulting and business services to the affiliated healthcare practitioner.
